Title IX Discrimination and Harassment Reporting Form

This form is designed for anyone (students, faculty, staff, visitors and guests, etc.) to report incidents of harassment and discrimination to Lackawanna College, including but not limited to:

  • Sexual harassment
  • Sexual misconduct
  • Relationship violence
  • Retaliation
  • Stalking
  • Bullying
  • Hazing

Do not use this form to report events presenting an immediate threat. If you require emergency assistance, please contact Lackawanna College Public Safety at 570-961-7899 or call 911.

Lackawanna College’s Title IX Coordinator (or designee) receives and reviews all complaints and routes them to the appropriate Lackawanna College administrator for investigation. Questions and concerns can be directed to TitleIX@Lackawanna.edu

This form can be used by any person making a report on their own behalf who wishes to remain anonymous. It can also be used to anonymously file a report on behalf of another person. Lackawanna College will investigate every report to the extent possible. However, it is important to understand that if you choose to file an anonymous report, you may limit Title IX’s ability to remedy the problem. Lackawanna College will make every effort to protect the privacy of all involved, but the information you provide in completing this form is not confidential and may be shared as necessary for Title IX staff to take appropriate action.
